(Note: To speed up the registration process, you may download a student and parent/school contract from the website and fill it out prior to the registration appointment.)

Please bring the following to your appointment:

• Completed registration application (if you filled it out in advance)
• Copy of Birth Certificate
• Social Security Card
• Blue Health Record (from previous school or it can be gotten from the health department)
• Student’s IEP if applicable (McKay students)
• Tax Return from 2010 if applying for FTC Scholarship
• If grade 8 through 12, the student must be interviewed with the parent at the school
• During the registration process all enrollment documents will be completed
• An anticipation of great things to come

If your child is a McKay Scholarship or FTC Scholarship Student, be sure to withdraw them from their previous school as soon as possible after registration.
